Federal regulations require you to verify certain statements or claims you make on your application for admission. Below is a list of things that are required WITH your application. Income, public assistance, banking and child support information must be CURRENT.

DRIVER’S LICENSE OR OTHER CURRENT PICTURE ID
FOOD BENEFITS + STATE SUPPLEMENT – STATEMENT FROM DHS
SOCIAL SECURITY CARDS
PROOF OF PREGNANCY – STATEMENT FROM DR.
BANK STATEMENT
MEDICAL EXPENSES – DISABLED OR 62+ – STATEMENT FROM DR.’S AND/OR PHARMACY
CHILD SUPPORT – PRINTOUT FOR THE LAST 6 MONTHS
PUBLIC ASSISTANCE – CASH ASSISTANCE
INCOME – WAGES; SSI; SOCIAL SECURITY; CHILD SUPPORT/ ALIMONY; FIP; UNEMPLOYMENT; PENSION; AND OTHER HOUSEHOLD INCOME

3. This form will be returned to you to complete if you do not answer all questions, and sign all authorization.
4. You must notify the Housing Commission in writing of any changes in address or family status.
